EMPLOYER SUPPLEMENTED BENEFITS

Angel Staffing provides you the opportunity to participate in the employer sponsored Group Health Insurance Plan offered through Principal Financial Group.

Angel Staffing will pay 50% of employee only medical premium when you meet the required full time status. Full time status is defined as a minimum of 36 hours per week or 80 hours in a 14 day pay period. One grace period of a single pay period is allotted per employee per year. All contract personnel taking PTO and/or vacation will be covered in full for those time periods.

LIFE

Angel Staffing will provide employees with $50,000 life and $50,000 accidental death and dismemberment insurance at no cost. To be eligible, you must work at least 36 hours per week.

Exam $10 co-pay annually
Glasses: $15 co-pay (frames every 24 months, lenses annually)
Contact Lenses: plan pays up to $105 every 12 months

A voluntary plan that reimburses you/your beneficiary for expenses incurred due to your death, dismemberment, or injury caused by a covered accident. Benefits are payable in addition to other coverage. Please refer to the Colonial brochure or contact Human Resources for detailed information.

Short Term Disability www.colonial.com
A voluntary plan that provides a monthly benefit should you become disabled and unable to work. Benefits will begin on the 8th consecutive day of illness and can continue for up to 3 years. For detailed information please contact the Human Resources Department.
